Our Process

Step 1 – Referral or Enquiry

A referral can be submitted through our online referral form, by phone, or via email. Our customer service team will review the information and confirm key details with the participant, family member, or referrer.

Step 2 – Appointment Scheduling

Once the referral is received, we will contact the participant to arrange a suitable appointment time. We aim to provide appointments as soon as possible depending on service availability and location.

Step 3 – Initial Assessment

A clinician will attend the participant’s home to complete a comprehensive assessment. This may include reviewing mobility, daily function, safety risks, communication, nutrition, or other relevant needs depending on the service required.

Step 4 – Therapy Plan and Recommendations

Based on the assessment findings, the clinician will develop a personalised therapy plan. This may include therapy sessions, home programs, equipment recommendations, or home modification advice to support the participant’s goals.

Step 5 – Ongoing Support and Review

Our team will continue to work with the participant, family members, and support providers to deliver therapy sessions, monitor progress, and provide reports when required for care planning or funding reviews.

We Come to You! Our mobile allied health team provides services across Sydney metropolitan suburbs, supporting participants in their homes and community settings. Alternatively, participants can visit one of our hubs for guided exercise and training with our experienced clinicians.

Referrals can be submitted through our secure online referral form. Once received, our customer service team will review the information, contact the participant when required, and arrange the initial appointment as soon as possible.

From therapists providing care in the home to our customer service and administrative teams coordinating appointments, everyone plays a role in supporting our participants’ safety, independence, and wellbeing.
