The Commonwealth Home Support Programme (CHSP) provides entry-level support services for older Australians, helping them maintain their independence, safety, and wellbeing within their home and community. The program focuses on early intervention and maintaining independence, helping older people stay active and engaged while reducing the risk of functional decline.
CHSP services are typically arranged through My Aged Care, where an assessment is completed to determine eligibility and the types of services that may benefit the individual.

Referrals can be submitted through our secure online referral portal.
We welcome referrals from Support at Home providers, CHSP providers, care partners, families, and participants seeking allied health support at home.
Once the referral is received, our customer service team will review the information and contact the participant or referring provider to schedule the first appointment.
