Commonwealth Home Support Programme (CHSP)
The Commonwealth Home Support Programme (CHSP) provides entry-level support to help older Australians remain safe, independent, and connected while living at home. Super Rehab works alongside CHSP providers to deliver practical allied health services that support mobility, safety, daily function, and overall wellbeing.
Our mobile clinicians deliver services in the home and community, ensuring supports are relevant, achievable, and aligned with the client’s goals and care plan.
Who CHSP Services Support
CHSP services are designed for older people who need entry-level support to remain living independently at home and in their community.
CHSP participants maybe someone who:
- Experience reduced strength, balance, or mobility
- Are at risk of falls or reduced confidence moving at home
- Need support to maintain independence in daily activities
- Are recovering after illness, injury, or hospitalisation
- Live with chronic health conditions impacting function
- Require guidance to remain active and safe at home
How We Work with CHSP Providers
Super Rehab works closely with CHSP providers to ensure services are coordinated, compliant, and aligned with client goals.
We provide:
- Timely assessments and service delivery
- Clear communication and service updates
- Functional recommendations to support care planning
- Documentation to support ongoing service coordination
GEAT2GO Funding Support
Our Occupational Therapists can assess functional needs and recommend assistive equipment to improve safety and independence at home.
Where eligible, we can assist with GEAT2GO funding applications to support access to essential equipment.

- In the community environment
- Via telehealth when appropriate
