Restorative Care Pathway

What is the Restorative Care Pathway?

The RCP is a short-term, up to 16-week, clinically focused program designed to help older people maintain or regain function, reduce decline and delay higher levels of care. It offers:

  • Intensive allied health and/or nursing input
  • Goal-centred reablement strategies
  • Multidisciplinary allied health work
  • Regular monitoring and progress reviews
  • A focus on meaningful, measurable outcomes

Eligibility

An older person may be eligible if:

  • The person is aged 65+, or 50+ for Aboriginal and Torres Strait Islander people
  • The person has a short-term functional need that is likely to improve with allied health support
  • The person is motivated and able to participate actively in a restorative program
  • The person recently experienced a decline due to falls, illness, surgery, or health changes


RCP can be accessed alone, or alongside ongoing Support at Home services.

RCP and Ongoing Support at Home Services

Participants who already have an ongoing SaH classification can access RCP concurrently. This means:

  • SaH participants can continue using their quarterly SaH budget
  • RCP provides additional short-term funding on top
  • RCP services must be clinically focused and complementary to ongoing services
  • RCP allows older people to receive an intensive functional boost without disruption in their ongoing supports

Funding

Each RCP episode includes:

  • Up to 16 weeks of allied health/nursing supports
  • One unit of funding (up to $6,000) for that episode
  • Participants may receive up to 2 units per year
    • Two separate episodes, OR
    • Two units combined into one episode (requires MAC assessor approval)

Bonus point: Care management for RCP is funded from the RCP budget, not from pooled Support at Home care management funds

Access to AT-HM Scheme

Participants in RCP may simultaneously access the Assistive Technology & Home Modifications (AT-HM) Scheme, which is funded separately from both Support at Home and RCP budgets:

  • RCP participants can receive low, medium or high-tier AT and low or medium-tier HM, but not high-tier HM unless already approved under an ongoing SaH classification
  • AT-HM approval is based on needs identified at assessment
  • AT-HM items is on the AT-HM scheme list

Program Duration

RCP is designed as a continuous program and generally runs for up to 16 consecutive weeks. 

  • Unlike the preceding STRC Program, there are no leave provisions in RCP
  • If the participant is away, hospitalised, or unable to engage, the clock continues
  • The participants would start episodes when they are ready to fully engage

How Super Rehab Supports Participants

We are here to support older persons restore function, rebuild confidence, and delay future care needs, with intensive, short-term allied health support, including:

  • Working collaboratively with providers to identify participants who are suitable for RCP
  • Rapid-response allied health intervention, prioritising timely assessments and therapy
  • Regular outcome reviews to monitor progress and adjust goals as needed
  • Exit planning and seamless transition into ongoing Support at Home services when required

Our Multidisciplinary Allied Health Team

Super Rehab delivers coordinated care through a multidisciplinary allied health team, including:

  • Occupational Therapy
  • Physiotherapy
  • Exercise Physiology
  • Dietetics
  • Speech Pathology
  • Podiatry
  • Allied Health Assistants


Our clinicians collaborate with care partners, GPs, nurses, and families to ensure therapy is coordinated and aligned with the participant’s goals.

Service Delivery Options

Super Rehab allied health services can be delivered in a range of settings, depending on the participant’s needs and clinical requirements:

  • In the participant’s home
  • In the community environment
  • Group exercise sessions at our hubs or community venues
  • Via telehealth when clinically appropriate

Working with Care Partners and Providers

Under the RCP program, each participant is supported by a clinical care partner who coordinates services and ensures care remains aligned with the participant’s goals and support plan.


Super Rehab works closely with care partners and providers to deliver:

  • Hub-based or home-based allied health services
  • Clear communication and collaborative care
  • Multidisciplinary team support
  • Experienced in aged care and disability funding schemes


This collaborative approach helps ensure participants receive coordinated, goal-focused support throughout their restorative care program.

Referral and Next Steps

Referrals can be submitted through our secure online referral portal.


We welcome referrals from Support at Home providers, CHSP providers, care partners, families, and participants seeking allied health support at home.


Once the referral is received, our customer service team will review the information and contact the participant or referring provider to schedule the first appointment.
