The End-of-Life Pathway is a short-term support pathway within the Australian Government’s Support at Home program designed to assist older people who have been diagnosed with a life expectancy of three months or less and wish to remain at home. It provides additional funding to ensure the participants receive timely and coordinated services to support comfort, safety, and dignity during the last months of life, while working alongside medical teams and palliative care services.

An older person may be eligible if:

Participants receive up to $25,000 of funding over a 12-week period to access in-home care and support services. The funding can be used up to 16 weeks if required. If additional support is required, services may continue while an urgent review is undertaken to transition the participant to an ongoing Support at Home classification.

Referrals can be submitted through our secure online referral portal.
We welcome referrals from Support at Home providers, CHSP providers, care partners, families, and participants seeking allied health support at home.
Once the referral is received, our customer service team will review the information and contact the participant or referring provider to schedule the first appointment.
