Six disciplines.
One coordinated team.
Evidence-based rehabilitation delivered at home — focusing on mobility, strength, falls prevention, and post-surgical recovery. Our physios work with participants in their real environment, on their own turf.
Thorough home assessments, assistive technology prescription, home modification design, SIL/SDA assessments and NDIS functional capacity reports — all at the participant’s home.
Tailored exercise programs to improve function, manage chronic conditions, and build the strength and confidence participants need to remain independent at home and in the community.
Assessment and treatment of communication and swallowing difficulties, including AAC device support and mealtime management plans — delivered in the participant’s home and dining environment.
Comprehensive nutritional assessments, personalised meal planning, and management of complex dietary needs — conducted in the participant’s own kitchen for practical, real-world recommendations.
Supervised therapeutic support in the home — implementing programs designed by our clinical team to build independence in daily living, personal care, and community participation.
Clinical programs
built for your participants.
An OT-led intervention for participants living with cognitive decline or impairment. Focuses on promoting independence, engagement and quality of life — with a personalised activity plan built around each participant’s history, hobbies and interests.
Structured group exercise programs delivered by our Exercise Physiologists — designed to improve strength, balance and mobility while fostering social connection and community participation for NDIS and aged care participants.
Practical manual handling training for carers, support workers and healthcare staff — delivered by our clinical team to reduce workplace injury risk and ensure safe, confident participant handling techniques.
Built around
better outcomes.
