At Super Rehab we support older Australians to live safely and independently at home. With the transition to the Support at Home program, we’ve expanded our services to include the Restorative Care Pathway — delivering short-term allied health interventions to help restore function and wellbeing.
We work across key funded home-care programs, including:
Under the new Support at Home Program, ongoing services will be funded through eight classification levels. These levels replace the traditional Home Care Package (HCP) levels, and are designed to provide a more accurate match between a participant’s needs and their funding. Participants will be assigned to one of the 8 classifications after their aged care assessment.
Each classification comes with a quarterly budget that can be used flexibly across services such as allied health, daily living support, cleaning, personal care, transport, and social participation — according to the client’s goals and assessed needs.
When the Support at Home program commences, people who were previously receiving a Home Care Package (HCP) will not immediately move into the new 8-level classification system. Instead, they will enter one of four transitioned classifications that directly reflect their former HCP level.
This ensures that older people experience no disruption to their services or funding while the new system rolls out.
If a person was receiving a certain Home Care Package level before the transition, Support at Home will continue to fund them at the same annual value, there is no reduction in their allocation during the transitional period.

The Restorative Care Pathway (RCP) is a short-term, intensive program under the Support at Home framework designed to help older people regain function, improve confidence and maintain independence at home. Over a period of up to 16 weeks, participants receive coordinated allied health support, regular monitoring and a personalised goal-focused plan. Super Rehab delivers multidisciplinary RCP services across Sydney and works closely with providers, care partners and families to ensure seamless, reablement-focused care.
The Assistive Technology and Home Modifications (AT-HM) scheme provides eligible older people with the equipment and home modifications they need to live safely and independently at home. Under the Support at Home program, AT-HM funding is based on assessed need and supports items such as mobility aids, bathroom modifications, rails, ramps and other functional equipment. Super Rehab’s clinicians assess, prescribe and recommend appropriate AT-HM solutions, working closely with providers and families to ensure each item or modification aligns with the participant’s goals and environment.
