The Support at Home provides funding and services to help older people remain living safely and independently in their own homes. It supports participants to access a range of services such as personal care, allied health, domestic assistance, assistive technology, and home modifications based on their assessed needs and goals.
Support at Home replaced the previous Home Care Packages (HCP) and Short-Term Restorative Care (STRC)programs, and brings together ongoing care services, short-term restorative programs, and assistive technology funding into a single system designed to provide more flexible, coordinated support for older Australians.
Under the new Support at Home program, ongoing services are funded through 8 classification levels and are designed to better match a participant’s needs with the level of funding provided.
Each classification includes a quarterly budget, which can be used flexibly for services such as:
When the Support at Home program commenced, people who were already receiving a Home Care Package (HCP) did not immediately move into the new eight-level classification system.
Instead, they were placed into four transitional classifications, which reflect their previous Home Care Package level. This approach ensures that participants experience no disruption to their services or funding during the transition period.
Participants will continue to receive funding at the same annual value while the new system is gradually implemented.

The Restorative Care Pathway (RCP) is a short-term program under the Support at Home framework designed to help older people regain function, build confidence, and maintain independence at home.
Over a period of up to 16 weeks, participants receive coordinated allied health support, regular monitoring, and a personalised goal-focused care plan.
Super Rehab delivers multidisciplinary restorative care services across Sydney, working closely with providers, care partners, and families to ensure coordinated and reablement-focused care.
The Assistive Technology and Home Modifications (AT-HM) Scheme provides eligible older people with equipment and home modifications that help them live safely and independently at home.
Funding is based on assessed need and may support items such as:

Referrals can be submitted through our secure online referral portal.
We welcome referrals from Support at Home providers, CHSP providers, care partners, families, and participants seeking allied health support at home.
Once the referral is received, our customer service team will review the information and contact the participant or referring provider to schedule the first appointment.
