From referral to
first visit.
A referral can be submitted through our online referral form, by phone, or via email. Our customer service team will review the information and confirm key details with the participant, family member, or referrer.
Online Referral FormOnce we receive your referral, our customer service team will review the details and send you a confirmation email within 24 business hours. This email will include a PDF copy of the completed referral form as submitted through our online referral portal — for your records and the participant’s file.
Once the referral is received and confirmed, we will contact the participant to arrange a suitable appointment time. We aim to schedule appointments as soon as possible depending on service availability and location. If the referrer or a family member would like to be present at the assessment, we will do our best to accommodate their requested date and time.
Get in Touch With UsA clinician will attend the participant’s home to complete a comprehensive assessment. This may include reviewing mobility, daily function, safety risks, communication, nutrition, or other relevant needs depending on the service required. The initial assessment usually takes 1–2 hours, depending on the participant’s care and functional needs and complexity.
Following the initial assessment, we will prepare a detailed report and send it to the referrer. For aged care initial assessments, reports are typically completed within 5 business days. NDIS reports or complex aged care cases may take longer — our team will always communicate with referrers proactively so you are kept informed throughout the process.
Based on the assessment findings, the clinician will develop a personalised therapy plan. This may include therapy sessions, home programs, equipment recommendations, or home modification reports to support the participant’s goals.
Our team will continue to work with the participant, family members, and support providers (such as care partners and support coordinators) to deliver therapy sessions, monitor progress, and provide reports when required for care planning or funding reviews.
When a participant has reached their goals or chooses to conclude their services with Super Rehab, our clinicians will prepare a formal clinical handover or discharge summary. This document is provided to support any ongoing services the participant may be transitioning to, and to keep their GP and care team informed. Our aim is to ensure continuity of care and a smooth, well-communicated conclusion to our involvement.
