What is the End-of-Life Pathway?
The End-of-Life Pathway is a short-term support pathway within the Support at Home program designed to assist older people who have been diagnosed with a life expectancy of three months or less and wish to remain at home.
It provides additional funding to ensure participants receive timely and coordinated services to support comfort, safety, and dignity during the last months of life — while working alongside medical teams and palliative care services.
What the End-of-Life Pathway provides
Participants receive up to $25,000 over 12 weeks to access in-home care and support services. Funding can extend to 16 weeks if required. If additional support is needed, services may continue while an urgent review is undertaken to transition the participant to an ongoing Support at Home classification.
Funding can be used to access a range of in-home services, including personal care, nursing support, allied health services, assistive technology and home modifications through the AT-HM Scheme, domestic assistance, and care management.
How Super Rehab supports participants
Super Rehab provides allied health services that support comfort, safety, and function during end-of-life care. Our clinicians work closely with care partners, families, and healthcare teams to ensure services are coordinated and aligned with the participant’s needs and wishes. Our goal is to help participants remain comfortable, supported, and safe within their home environment.

- Mobility support and safe transfers
- Positioning and pressure care strategies
- Equipment assessment and prescription
- Home environment safety recommendations
- Communication and swallowing support
- Nutritional guidance and comfort-focused care
- Support for carers and families assisting at home

Under the End-of-Life Pathway, each participant is supported by a care partner who coordinates services and ensures care remains aligned with the participant’s needs. Super Rehab works closely with care partners and providers to deliver coordinated and responsive care during this important time.
