What is Support at Home?
Support at Home replaced the previous Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programs. It brings together ongoing care services, short-term restorative programs, and assistive technology funding into a single, more flexible system for older Australians.
Participants can access a wide range of services based on their assessed needs and goals — including allied health, personal care, domestic assistance, assistive technology, and home modifications.
Eight classification levels, matched to need.
Under Support at Home, ongoing services are funded through 8 classification levels. Each level includes a quarterly budget that can be used flexibly across allied health, personal care, domestic assistance, transport, and social participation.
Restorative Care Pathway
The Restorative Care Pathway is a short-term program designed to help older people regain function, build confidence, and maintain independence at home. Participants receive coordinated allied health support, regular monitoring, and a personalised, goal-focused care plan.
Super Rehab delivers multidisciplinary restorative care across Sydney, working closely with providers, care partners, and families to ensure coordinated, reablement-focused care.
AT-HM Scheme
The AT-HM Scheme provides eligible older people with equipment and home modifications that help them live safely and independently. Funding supports items such as mobility aids, bathroom modifications, handrails, ramps, and functional equipment for daily activities.
Super Rehab clinicians assess, prescribe, and recommend appropriate AT-HM solutions. We work closely with providers, families, and builders to ensure each recommendation aligns with the participant’s goals and living environment.
Multidisciplinary care, delivered at home.
Super Rehab delivers coordinated care through a multidisciplinary team. Our clinicians collaborate with care partners, GPs, nurses, and palliative care teams to ensure services complement existing supports.

In the participant’s home
Our most common setting — care delivered where the participant is most comfortable and where functional goals are most meaningful.
In the community
Therapy in community environments relevant to the participant’s goals — shops, parks, transport, or community centres.
Group exercise at our hubs
Group sessions at our Petersham or Homebush hubs, or at community venues — supporting social engagement alongside clinical goals.
Via telehealth
When clinically appropriate — supporting participants who are unable to attend in person or who prefer remote consultations.

Under Support at Home, each participant is supported by a care partner who coordinates services and keeps care aligned with the participant’s goals. Super Rehab works closely with care partners and providers to deliver seamless, goal-focused support.
