About the Program

What is the Restorative Care Pathway?

The Restorative Care Pathway (RCP) is a short-term, up to 16-week program designed to help older people maintain or regain function, reduce decline, and delay higher levels of care. It is clinically focused, goal-centred, and built around measurable outcomes.

RCP can be accessed alone, or alongside ongoing Support at Home services.

An older person may be eligible if they:

  • Are aged 65+, or 50+ for Aboriginal and Torres Strait Islander people
  • Have short-term needs likely to improve with allied health support
  • Are motivated and able to actively participate
  • Experienced decline due to falls, illness, surgery, or health changes

Funding

What each RCP episode includes

Each RCP episode provides up to 16 weeks of allied health and/or nursing support, with one unit of funding up to $6,000. Participants may receive up to 2 units per year — as two separate episodes, or combined into one longer episode with MAC assessor approval.

For participants already on Support at Home, RCP provides additional funding on top of their quarterly SaH budget. Care management is also funded from the RCP budget.

Program timing: RCP runs continuously with no leave provisions. The 16-week clock continues even if the participant is away or hospitalised. Participants should begin when ready to fully engage.

Assistive Technology & Home Modifications

Access to the AT-HM Scheme during RCP

Participants in RCP may simultaneously access the AT-HM Scheme, which is funded separately from both Support at Home and RCP budgets.

  • RCP participants can access low, medium, or high-tier AT
  • RCP participants can access low or medium-tier Home Modifications — high-tier HM requires approval under an ongoing SaH classification
  • AT-HM approval is based on needs identified at assessment and items must appear on the approved AT-HM scheme list

Collaborative care throughout RCP

Under RCP, each participant is supported by a clinical care partner who coordinates services and keeps care aligned with the participant’s goals and support plan. Super Rehab works closely with care partners and providers to ensure services are coordinated, clinically focused, and goal-driven throughout the program.
