What is CHSP?
The Commonwealth Home Support Programme (CHSP) provides entry-level support services for older Australians, helping them maintain their independence, safety, and wellbeing within their home and community. The program focuses on early intervention, helping older people stay active and engaged while reducing the risk of functional decline.
CHSP services are typically arranged through My Aged Care, where an assessment is completed to determine eligibility and the types of services that may benefit the individual.
Services available under CHSP
CHSP funding can be used to access a range of services based on assessed need, including:
- Allied health services — occupational therapy, physiotherapy, and more
- Domestic assistance and household tasks
- Personal care support
- Transport assistance
- Social support and community activities
How Super Rehab supports CHSP participants

- Exit planning and seamless transition into ongoing Support at Home services when required
Our Occupational Therapists can assess a participant’s functional needs and recommend assistive equipment to improve their safety and independence at home. Where eligible, we can assist with GEAT2GO funding applications, helping participants access essential equipment that supports safer daily living.

Multidisciplinary care, delivered at home.
Super Rehab delivers coordinated care through a multidisciplinary allied health team. Our clinicians collaborate with care partners, GPs, nurses, and palliative care teams to ensure services complement existing supports.
How and where we deliver services
In the participant’s home
Our most common setting — care delivered where the participant is most comfortable and functional goals are most meaningful.
In the community
Therapy in community environments relevant to the participant’s goals — shops, parks, transport, or community centres.
Group exercise at our hubs
Group sessions at our Petersham or Homebush hubs, or at community venues — supporting social engagement alongside clinical goals.
Via telehealth
When clinically appropriate — supporting participants who are unable to attend in person or who prefer remote consultations.
Collaborative care that puts participants first.
Super Rehab works closely with CHSP care partners and providers to ensure services are coordinated, compliant, and aligned with each participant’s goals.
