This is a friendly reminder that the deadline for submitting AT-HM Scheme data collection for HCP participants transitioning to the SaH is fast approaching. All data must be submitted by 31 January 2026 in line with the department guidelines. From February 2026, the department will close data collection submissions and finalise in-hand requests .

Key Points to Remember
- If participants have already been allocated an AT-HM tier through a previous data collection process, Support Plan Review (SPR), or reassessment, they are not eligible for an additional funding tier through the current AT-HM data collection process.
- The AT-HM data collection process can be used to request AT-HM funding up to $15,000.
- For AT requests over $15,000, you can complete this form AT over $15,000 Application
and email it to ATHighTierClaims@health.gov.au

To support timely compliance, Super Rehab has availability this month to complete:
- Initial Occupational Therapy Assessment
- Comprehensive report including AT and/or HM recommendations
- Formal quotations required for data collection submissions
Please send any referrals or quote requests as soon as possible so we can finalise documentation before the 31 January deadline.
What happens from February 2026?
From 1 February 2026, the AT-HM data collection process will cease for transitioned HCP participants. Instead, providers must raise a SPR for any participants requiring additional funding for AT-HM under SaH.
