If you wish to give us feedback, great, we would love to hear that! You can talk to Margaret (Super Rehab Health Service Manager) on 0411 577 140 or send an email to margaret@superrehab.com.au

If you are not happy with the provision of therapy services and wish to make a complaint, you can talk to Margaret (Super Rehab Health Service Manager) on 0411 577 140 or send an email to margaret@superrehab.com.au You can also ask a family member, friend, or advocate to help you make a complaint. 

If you are an NDIS participant and you are not satisfied, or do not want to talk to our nominee listed above and wish to make an anonymous complaint, a complaint can be made by:


  • Written letters to Super Rehab posted to PO Box 608 Leichhardt 2040


  • Completing the Feedback and Complaint form anonymously on our website https://superrehab.com.au/feedback-and-complain (please see the below contact form, you can enter info@superrehab.com.au in the email section if you wish to remain anonymous)


  • Phoning the NDIS Commission: 1800 035 544 (free call from landlines) or TTY 133 677. Interpreters can be arranged.


  • Phoning the NDIS National Relay Service and ask for 1800 035 544.


  • Completing a complaint contact form from NDIS Commission website www.ndiscommission.gov.au
