We are working closely with home care providers and we have been providing high quality allied health services to the clients. Our therapists are able to complete comprehensive assessments and reports to ensure that all of clients' physical and functional needs are considered in the home care package.

At Super Rehab, each assessment goes far beyond a surface-level examination or the initial referral reasons. Our clinicians assess the clients, their environment, and daily activities, considering essential factors like safety, accessibility, and independence. This approach leads to fully personalised recommendations and plans which align with the clients’ lifestyle and enhances their quality of life.
Through extensive assessments and thorough discussions with the clients and their families, we help prevent unnecessary follow-up sessions, treatments, or additional expenses. We ensure our reports are precise, detailed and actionable, addressing current needs while also anticipating future requirements. This proactive, preventative approach saves clients money over time, allowing them to invest in long-term health and well-being.
Our assessments are enriched by insights from a diverse team of allied health professionals. For example, a dietetics assessment may include recommendations from an exercise physiologist for a balanced nutrition and exercise plan, or a physiotherapy assessment may draw from occupational therapy expertise to create a safe and hazard-free home environment.
Each assessment includes detailed reports that clients and their care managers can rely on to make informed decisions. Our clinicians also conduct regular follow-up reviews to update treatment plans as clients progress. For instance, our physiotherapists and exercise physiologists perform functional outcome assessments every eight weeks, tracking improvements and setting new goals to support continued growth and achievement.

General Referral is intended for clients who are 65 years old and above, and who are recipients of aged care funding, i.e. HCP, CHSP and STRC, or privately funded.
